Take C.D.S. Exam
To take your C.D.S. exam, please follow the following four steps.
Step 1: Locate your CDS exam Voucher Code.
Note: If you already have a C.D.S. voucher code from your employer, please go to Step 2.
Individuals (purchased for yourself)
- Go to www.hcinteractive.com/myaccount.
- Log in to your HCI account with the user ID and password you created when you purchased this product.
- Scroll down and locate your CARES Dementia Specialist Exam Voucher purchase.
- Write down your Voucher Code.
- Click on the link below the code, and then log in to begin your CDS exam.
Administrators (purchased for your staff members)
- Go to www.hcinteractive.com/myaccount and log in to your HCI account with the user ID and password you created when you purchased this product.
- Scroll down and locate your CARES Dementia Specialist Exam Voucher purchase.
- Write down the Voucher Code.
- Give the Voucher Code to your staff member.
- Have your staff member go to www.hcinteractive.com/redeemCDS, enter the Voucher Code, and begin the CDS exam.
Step 2: Pass the CDS exam with at least 90% correct.
Go to www.hcinteractive.com/redeemCDS, enter the Voucher Code, and begin the CDS exam. Pass the exam with at lest 90% correct.
If you don't pass, you can re-take the exam. Any notes you take when completing the programs can be used on the credentialing exam. The exam is divided by program, so you can reference notes from each program for each section of the exam.
Step 3: Print out your CDS exam completion certificate.
Step 4: Submit your documentation for approval.
Once you complete all CARES courses and pass the CARES credentialing exam, you must submit your documentation for approval to officially become a CARES Dementia Specialist. Print the form below, fill it out, attach copies of your training certificates and C.D.S. exam certification, and submit to HealthCare Interactive for approval. Once we verify you have met all of the requirements, we will e-mail you with an official confirmation and will mail your recognition package to you.
