CARES^® Dementia Certification^™

This training program includes CARES Dementia Certification from HealthCare Interactive. Certification is valid for two years after you pass the certification exam.

Instructions:

• Complete all modules of this CARES^® training program.
• Click on the Take Certification Exam button.
• Pass the exam with a score of 80% correct or higher. (You may take the exam as many times as you wish. You may also use any notes on the exam.)
• On the course progress page, if you have passed your exam, click the “Print Certification Certificate” button.

Alzheimer’s Association^® essentiALZ^® Certification

This training program includes essentiALZ certification from the Alzheimer’s Association that is valid for two years after you pass the certification exam.

Instructions

1. Complete this training program, and print your completion certificate.
2. Follow the instructions below your certificate to take the essentiALZ exam.
3. Take the exam within 6 months of completing this program.
4. Pass the exam with a score of 90% correct or higher. You will have 2 attempts to pass the exam. You may use your worksheets and notes during the exam.
